Appointment card template

A blank appointment card template for any business

Each card has space for the client, date, time, provider, address, and phone number, plus a reminder to call at least 24 hours in advance to cancel or reschedule. Salons, clinics, dentists, therapists, and repair services can all use the same layout. Download the Word file to add your business name and logo, or print the PDF as it is and fill the cards in by hand.
